Job description:
Patient Assessment & Care Planning
- Conduct comprehensive functional assessments (physical, cognitive, psychosocial)
- Develop individualized therapy plans based on patient condition
- Set measurable rehabilitation goals
Therapy Intervention
- Provide therapy for patients with conditions such as:
- Train patients in Activities of Daily Living (ADLs)
- Improve fine motor skills, coordination, and cognitive function
Rehabilitation & Functional Training
- Facilitate recovery through structured therapy sessions
- Support return-to-work and functional independence programs
- Conduct group therapy where applicable
Assistive Devices & Adaptation
- Recommend and train use of assistive devices (splints, adaptive tools)
- Suggest home/workplace modifications for safety and accessibility
Documentation & Reporting
- Maintain accurate patient records and progress notes
- Prepare discharge summaries and rehabilitation reports
- Ensure documentation as per NABH standards
Multidisciplinary Coordination
- Work closely with doctors, physiotherapists, nurses, and psychologists
- Participate in clinical rounds and case discussions
Patient & Family Education
- Educate patients and caregivers on therapy techniques and home care
- Provide guidance for long-term rehabilitation
Quality & Safety Compliance
- Follow hospital protocols and infection control practices
- Ensure patient safety during therapy sessions
